She was predeceased by: her husband Leslie; her parents, Norris Hart and Helen Hart; her sister Alice Hart; and her stepsons, Allen and Dennis Chappell.
She is survived by: her daughters, Lisa Craft (Dennis) and Lori Chappell (Daryl Motes); her grandsons, Darren Fath (Alysha) and Cody Fath; her great grandchildren, Connor, Bailey and Easton Fath; and her stepgrandchildren, Teri Olds (David), Sid Chappell (Lisa) and Carlos Montague.
A funeral service will be held at the funeral home on Monday, October 19 at 11:00 a. m, with visitation from 10:00 to 11:00 a. m. In Phyllis' honor and to help prevent the spread of Covid-19, please observe the proper Covid-19 guidelines, including wearing a face mask.
