He was predeceased by: his parents, Lawrence Victor and Mildred Vernon Ballard (King); his wife Darlene Nation; and his daughter Denise Cox. He is survived by: his sons, David Jr. Ballard (Lisa) and Duane Ballard (Tammy); and his sister Delores "Dolly" Thibeaut. He is also survived by 7 grandchildren, 4 great-grandchildren.
Memorial donations in Mr. Ballard's name are suggested to the American Cancer Society.
