He was predeceased by: his parents, Joseph P. Jerzak and Florence A. Jerzak; his wife Mary Jerzak (Canfield); his siblings, Marion Jerzak, Ester Jerzak (Booth), Richard Jerzak and Donald Jerzak; his granddaughter Erika Jerzak; and his daughter-in-law Jacquie Jerzak.
He is survived by: his children, David Jerzak of Okemos, MI, Jim Jerzak (Jade) of Grand Ledge, MI and Ann Alcaraz of Ovid, MI.; his sister Beverly Sieczko; and his grandchildren, Skye, Alec Alcaraz, Will, Eli and Weston Jerzak.
