He was predeceased by: his parents, Fredrick Mayhew and Phyliss Mayhew. He is survived by: his wife Yvonne Mayhew; his children, Stephanie, Steven, Kristina, Megan and Katie; his siblings, Sharlyn Andrews, Cathy Mayhew, Bruce Mayhew, Larry Mayhew and Scott Mayhew; and his close friends, Nancy, Robert (Sandra), Heidi, Bandit and Cujo. He is also survived by 16 grandchildren; 5 great-grandchildren.
