He was predeceased by: his parents, Shirley Cook (Border) and Wayne "Bud" Cook; and his siblings, Steven and Cheryl. He is survived by: his nephew Wayne; and his uncle Velvin Border (Rita).

In lieu of flowers, the family is asking to consider memorial donations to the Capital Area Humane Society in memory of Jay.
