He is survived by: his brother John W. Perkins (Lorayn); his nephew Jeffrey W. Perkins (Cheryl); his niece Julie Hays (Andrew); his great-nieces, Victoria Hays and Eve Perkins; and his great-nephew Ryan Hays.
