He was predeceased by: his father Gary D. Crouse. He is survived by: his mother Leona M. Nye; his children, Chadwick Crouse (Christina), Christopher Crouse and Shana Crouse; his siblings, Pamela Smith, Lucinda Crouse, Rhonda Grantham (Thomas) and Gregory Crouse (Mary Steele); his grandchildren, Bentley Revilla, Deslynn Crouse, Jacob Crouse, Layla Teisan and Gianna Teisan; his former wives, Connie Smith and Patty Colegrove; and his caregivers, Patty Colegrove and Shana Crouse. He is also survived by several nieces, nephews, cousins, aunts and uncles.
