He was predeceased by: his parents, Henry Kuizema and Elsie Kuizema; his wife Jessica DeBoer; his daughter Diane; and his son-in-law Kock Seng Chee.
He is survived by: his daughters, Ellen Thomasma (Norm), Carole Valk (Albert) and Joan Chee; his son-in-law Mark DeHaan; his sister Jeanne Rypma; and his siblings-in-law, Dorothy Zylstra Sheler (Dale) and Ken Kwantes. He is also survived by 6 grandchildren, and 7 great-grandchildren.
In lieu of flowers the family suggests donations to Oakdale Park Christian Reformed Church.
