She is survived by: her husband Ronald Cusworth; her children, Ronald Cusworth, Robert Cusworth, Catherine Cusworth and Lynn Qualls (Jerry); her grandchildren, Carissa Olejnik (James), Jesse Burks, John Qualls (Jenny) and Melissa Qualls (Jeff Lelito); her great grandchildren, Jessa, Grace, John Jr., Jenna and Hazel; and her brothers, Carl DeSantis and Dennis DeSantis.
