He was predeceased by: his father Jack Sulka; his brother Mike Sulka; and his stepdaughter Brooke Gillespie. He is survived by: his sons, Christopher and Alex Sulka; his stepchildren, Blair Hegner (Travis), Brandon Gillespie (Danielle) and Brent Gillespie (Natalie); his siblings, Tom Sulka (Susie), Sue Evans, Diane Sulka and Steve Sulka; his nieces and nephews, Jennifer Houser, Danny Houser (Kristina), Sarah Houser, Katelyn Sulka, Madison Sulka and Scott Sulka; and his pet Boomer.

In lieu of flowers, those desiring may make memorial contributions to the charity of their choice, in memory of John.
