He was predeceased by: his parents, Ervin H. Lewis Sr. and Dawn M. Guernsey. He is survived by: his daughter Courtney Lewis; his grandchildren, Adaline and Molly Sabo; and his siblings, Brian Guernsey (Kerri), Richard Abraham, Jackie Zima, Tina Rix (Joe), Juanita Marcon (Doug) and Valerie Connor (Patrick). He is also survived by many nieces, nephews and friends.

Memorial contributions may be made to Ervin's grandchildren (checks payable to Courtney Lewis), c/o the funeral.
