He was predeceased by: his parents, Earl Middlebrooks and Agnes Middlebrooks (Morgan); his wife Grace Belle Curtis; his daughter Patricia Middlebrooks; his grandson Michael Ringel; his son-in-law Charles Huff; and his sister-in-law Beverly Middlebrooks.

He is survived by: his children, Donald E. Hose, Lois A. Hose, Jeffrey A. Hose, Susan J. Embler and Lori A. Huff; and his brother Roy Middlebrooks. He is also survived by twelve grandchildren and eight great grandchildren.
