He is survived by: his wife Donna Bolks; his children, Lynn (Jim Fields), Ken (Michelle Bolks), Rich (Maria Bolks) and Steve (Sonya Bolks); his great grandchildren, Jay, Dana Fields, Kylee, Makenzy, Markie, Ariel Bolks, Katie, Corey, Zach, Abby Bolks, Nick Bolks (Emily Owen), Kelsey and Josh Scott (Forrest); his brother Roger (Helen Bolks); and his siblings-in-law, Velma Wustman, Ruth, Bob Vander Kam and Minie Hoeksema. He is also survived by many nieces and nephews.
