He is survived by: his children, Wayne Formolo (Denise), Todd Formolo (Debbie), Jessee Formolo (Manda), Tracy VanBeek (Rick), Lee Formolo and James "Jimbo" Formolo; his brother Dan Formolo (Leanne); his grandchildren, Corey, Emily, Mia, Mario, Nikolas, Haley, Dane, Dylan, Ciara, Kaitlyn, Anthony, Meredith and Jessica; his nephews, Bryan, Tim, Jared and Joe; his sister-in-law Mary Lou Clement; and also, Todd Dario (Elisa), Tim Dario (Jenny) and Fran Formolo.
