She was predeceased by: her parents, Frank Formolo and Anna Formolo (Yuhassey); and her former spouse Merle Beauchamp. She is survived by: her daughters, Gail Beauchamp and Linda Gendron (Rick Gendron); her grandchildren, Jason Gendron (Sarah), Jared Gendron (Kristen) and Jamie Phillips (Skyler); and her niece Carol Curtis. She is also survived by many great-grandchildren.
The family asks that memorials be made to the American Cancer Society or the American Parkinson's Foundation in her memory.
