He was predeceased by: his children, Kristin and Eric; his parents, Martin Kooistra and Wilhelmina Kooistra; and his sisters-in-law, Bea Heerema and Laura Kooistra.
He is survived by: his wife Lynn; his children, Vickie, Jim Zylstra, Michelle and Doug VanderVeen; his grandchildren, Nathan, Sam, Drew Zylstra, Elijah and Zoey VanderVeen; his sister Donna; his brother-in-law Mike VanderPloeg; his brother Ken Kooistra; and his siblings-in-law, Frank Heerema, Rollie, Sue Bossenbroek, Harley and Kathy Bossenbroek. He is also survived by many loving nieces and nephews.
