He was predeceased by: his parents, Jay Sr. Bouwkamp and Elizabeth Bouwkamp; his in-laws, John and Anna Rietman; his brothers, Cornelius Bouwkamp (Doretta) and Jay Bouwkamp Jr.; and his siblings-in-law, Alvin Beukema, Ray Veltema, John Haveman (Jean), Jim Schipper (Henrietta), Louis Brower (Mary), Baby Jacob Rietman, Jake, Leona Rietman Zoerman Rietman (Floyd Zoerman) and Pete Rietman (Shirley).
He is survived by: his wife Alma Bouwkamp (Rietman); his children, John Bouwkamp (Vonnie), Joy Morren (John), Brenda Hoffman (John), Randy Bouwkamp (Lisa), Sandy Berens (Mark) and Scott Bouwkamp (Jenni); and his siblings, Emily Veltema, Betty Bosman (Case) and Ronald Bouwkamp (Marlene). He is also survived by 35 grandchildren; 35 great grandchildren.
