He is survived by: his wife Ann Kotalik (Knapp); his daughters, Ashley Kotalik, Heather Kotalik (Kelly Cleary) and Katelynn Kotalik (Tony Merenuk); his grandchildren, Mason Ohm, Marleigh Ohm, Kaeden Gazetti, Lincoln Cleary and Arianna Merenuk; his parents, Janet Kotalik and George Kotalik (Judy); and his siblings, Darcy Kotalik (Sandra), Gary Kotalik (Angie), Jeff Kotalik (Juli) and Julie Degayner (Dwayne).
