He is survived by: his children, Dylan Hulbert, Megan Hulbert, Kelly Hulbert and Brian Hulbert; his wife Dawn Chamberlain; his mother Ila Hulbert; his siblings, Amy White, Jerry Hulbert and Stacey Mason (Tim); his nieces and nephews, Sabrina, Cassandra, Gracie, Hannah "Jo", Zach, Kodey, Jonathan and Jerry Jr.; and his adoptive son Chris Bowers.
