He was predeceased by: his parents, Alan Lewis and Marian Lewis (Healy); his granddaughter Aria Lewis; and his sister Marianne Welker. He is survived by: his son Jacob Lewis (Dyallen Efendi) of Rogers, Arkansas; and his brothers, Alan Charles Lewis (Charmaine) of Fort Wayne, Neil Lewis (Jane) of Canton, Mark Lewis (Terri) of Wonder Lake, Steven Lewis (Judy) of Grove City, Donald Lewis (Michelle) of Tupelo and William Lewis (Gail) of Westerville, Ohio. He is also survived by several nieces and nephews.
