He was predeceased by: his brothers, Timothy Lyszak and Alfred Domanski. He is survived by: his wife Carol Ann; his children, Christine Belt (Brian), Michael Lyszak (Kathleen) and Patricia Dantes (Charles); his grandchildren, Daniel Belt (Beth), Kaitlin Lyszak, Jacquelin Dantes, Jenna Lyszak of Callie Belt, C. J and Michael Lyszak; and his sister Janet Faulkner. He is also survived by brothers & sisters-in-law, and his numerous close friends and extended family.

In lieu of flowers, donations are encouraged to Dan's family.
