He was predeceased by: his parents, Jerome Koscinski and Amelia Koscinski (Mostrag); and his sister Terri Duffey. He is survived by: his wife Carol Smith; his sons, Sam Koscinski (Kari) of Manistee and Christian Koscinski (Mary) of Annandale, Virginia; his grandchildren, Ethan, Nolan, Alivia, Mea, Alyssa and Cayden; and his siblings, Charles Koscinski (Sandy) of Oregon, Charlene Ellis of Colorado, Tim Koscinski of Texas and Paul Koscinski (Linda) of Texas. He is also survived by nieces, nephews, cousins, and friends.

The family prefers memorial contributions be made in Tom's name to the Capuchins, Cleveland Clinic, St. Jude's Children's Research Hospital, and the Children's Hospital of the King's Daughters, Norfolk, Virginia.
