He was predeceased by: his parents, Elmore Frederick and Katie Mae Boltz (Hargrove); and his siblings, Carrol Boltz, Kay Johnson, Gary Boltz and Fred Boltz.
He is survived by: his wife Diana M. Jachim; his brother Tom Boltz Sr.; and his brother-in-law Michael M. Jachim (Sue). He is also survived by nieces and nephews.
