He was predeceased by: his mother Barbara Forman (Aspinall); and his parents-in-law, Lyle Kneale and Joyce Kneale. He is survived by: his father Wayne Forman; his wife Kathy (Kneale); his children, Michael Forman (Amy) and Melanie Rice (Tyler); his grandchildren, Rose, Vada, Cecilia Forman and Kreighton Rice; his sister Susan VandeCar (Gary); and his brother-in-law Ken Kneale (Diane). He is also survived by nieces and nephews.
