He is survived by: his parents, Gabriel Martin and Ann Fields of Mentone; his brother Brantly Kreider; his grandparents, Jeremy Martin (Carrie) of Hastings, MI, Chasity Martin of Jacksonville, FL, Kathy Fields of Mentone, IN and Arthure "Red" Fields of Mentone, IN; and his great-grandparents, Diana Jurgenson (Scott) of Dowagiac, Kathy Martin of Coloma, Deborah Delk (Bruce) of Baroda, MI and Randy Moore of Jacksonville, FL. He is also survived by numerous aunts, uncles & cousins.
Memorial donations in Boone's name may be made to St. Jude Children's Hospital.
