He was predeceased by: his parents, Donald L Smith and Dolly Ann Smith. He is survived by: his spouse Jan Smith; his children, Dr. Beth Ann Smith and Jeffery Smith; and his brother Gregory Smith (Shelley). He is also survived by numerous nieces and nephews.
