She is survived by: her children, Maciej Kozak (Kimberly) and Anna Kozak (William Eubanks); and her grandchildren, Emmi, Tyler, Nathan, Bryan, Elizabeth, Katolina, Devon, Mary Grace, Daniel and Hayden. She is also survived by one great granddaughter; her family.
