He was predeceased by: his parent Katrina Bice. He is survived by: his wife Britt; his children, Cyress Velasquez-Mayhew (Cassaundra), Thaman Velasquez-Bice (Sara) and Alexis Velasquez-Mayhew (Deven Willard); his granddaughter Emeila Fraley Velasquez-Mayhew; his parent Katrina Mayhew; and his siblings, Shannon Mayhew, Carrie Sinclair (Danny), David Mayhew, Lisa Walter, Jessica Walter, Cara Mayhew, Danny Mayhew and Alec Mayhew. He is also survived by several nieces, nephews, and cousins.
