He was predeceased by: his mother Connie Collins; and his sister Pamela Collins. He is survived by: his wife Vicki Lewis Collins; his children, Christopher Collins, Candace Collins Wert (William Wert), Christina Collins Oskey (Aaron Oskey), Catherine Collins Feldmann (Gregory Feldmann), Caroline Collins Biscorner (Jason Biscorner), Jacob Collins Jr. (Vanessa Gonzales) and Claire Elisabeth Collins (Jane Lyszac); and his grandchildren, Samantha Collins, Lauren Fraser, Anna Fraser, Nadia Oskey and AJ Oskey.
