He is survived by: his wife JoAnn Setlock; and his children, Susan Grandy (Cole), Sandra Blake, Nancy Andersen (Mark), Cindy Blake, John Blake, Kim Weak (Gary), David Blake (Charlotte) and Dennis Blake (Deborah). He is also survived by 17 grandchildren; 17 great-grandchildren and a great great-grandchild.
