He is survived by: his parents, Jacob Connolly and Alison Connolly (Boka); his grandparents, Niki Boka, Darin, Tammy Boka, Scott and Amy Connolly; his great-grandparents, Jacque Rouse, Jackie Schmidt, Cathy Connolly, Mike and Cheryl Wildfong; his aunts and uncles, Bryan Boka, Emily, Zech Kosa, Jared, Maia Connolly, Zach Mulcahy and Alex Curry; his cousins, Cooper Schummer, Eloise Kosa and Willow Connolly; and his caregiver S. Mott of Children's Hospital. He is also survived by several great aunts and uncles and cousins.
