She was predeceased by: her husband Leonard Polakowski. She is survived by: her sons, Tim Polakowski (Meg), Greg Polakowski (Jessica) and Jeff Polakowski (Bonnie); her grandchildren, T. J., Jamie Zantop (Brian), Jeffrey, Jacob, Abby, Alison and Nichole; and her great grandchildren, Zoey and Emersyn. She is also survived by many nieces, nephews, and friends.
In lieu of flowers donations may be made to the American Cancer Society or St. Jude Children's Hospital.
