He is survived by: his sons, Tim Polakowski (Meg), Greg Polakowski (Jessica) and Jeff Polakowski (Bonnie); his grandchildren, T. J., Jamie Zantop (Brian), Jeffrey, Jacob, Abby, Alison and Nichole; his great grandchildren, Zoey and Emersyn; and his sister Florence Soens. He is also survived by many nieces, nephews, and friends.

In lieu of flowers donations in Leonard's name may be made to the American Diabetes Association.
