He was predeceased by: his parents, Jacob Adams and Esther Adams (Rood). He is survived by: his wife Barbara; his daughters, Darlene Querio (Walt) and Laura Smith (Chuck); his grandchildren, Jeremy Smith (Christin), Malinda Querio (Derick), Robert Smith (Amanda), Alyse Querio (Brian) and Thomas Smith (Jessica); and his great grandchildren, Blake, Avery, Vincent, Audrey, Emma, Everett, Tyler and Jenna. He is also survived by several nieces and nephews.

Memorial Contributions in Herbert's memory would be appreciated to Shriners Hospitals for Children.
