He was predeceased by: his parents, Donald Richard and Josephine Shook (Benhke); and his brother Loren Shook. He is survived by: his wife Delores Carol Leeck; his sons, Jeffrey Shook and Brian Shook (Denis Brown); his grandchildren, Emma, Cooper, Ryan, Paxton and Jordan; his great grandchild Charlotte; his sisters, Gloria Aldea and Rosemary Falter (Mike); and his aunt Betty Shook. He is also survived by several nieces and nephews.

Memorial Contributions in Forrest's memory may be made to Make a Wish Foundation-Michigan.
