He was predeceased by: his parents, Walter G. Polachek and Elizabeth Polachek (Schmucker); and his wife Nancy. He is survived by: his son Tracy Polachek (Autumn) of Engadine; his grandchildren, Caleb, Caitlynn Gehrett, Connor and Colton Polachek; and his siblings, Daniel Polacheck (Tracey), Judy Sadler (Kim) of Burton and Irene Clement of Ruston, LA. He is also survived by several nieces and nephews.
