He was predeceased by: his parents, Robert Bollinger and Cora Bollinger (Mayfield); and his siblings, Bobby, Denzel, Lenzel and Rose Lee Proffer.
He is survived by: his wife Helen; his children, Teresa King (Bill), Tim Bollinger and Ken Bollinger (Ken Carps); his grandsons, Alex Smith (Daniel), William Corey King and Wesley King; his great grandchild Beck Smith; and his brother Larry Bollinger (Jeanette). He is also survived by numerous nieces and nephews.
The family will receive friends at the funeral home starting at 11:00 A.M. For those desiring memorial contributions may be made to the American Lung Association or Susan G. Komen in memory of Jerry.
