She is survived by: her daughter Tammy Ballard (Duane); her grandchildren, Bryan, Alyssa, Alenah, Natasha, Andrew, Anthony, Nathan, Travis and Toni; and her sisters, Mary and Sally. She is also survived by many great grandchildren, nieces and nephews.

Memorial Contributions in Phyllis's name are suggested to the Durand Senior Care and Rehab Center.
