He was predeceased by: his parents, Pietro Provenzola and Mary Provenzola (Marano). He is survived by: his wife Shirley Ann O'Connor; his children, Mary Provenzola Fox, Robert Provenzola, Thomas Provenzola (Lola), Danielle Bulbuk (Tim) and Theresa Provenzola Smothers; and his grandchildren, Emily, Jesse, Maggie, Brent, Jason, Gina, Karly and Philip. He is also survived by 10 great-grandchildren.
Memorial donations in Frank's name may be directed to the American Cancer Society.
