She was predeceased by: her parents, Alvin Smith and Evelyn Flossie Smith (Garrett); her husband Kenneth Cook; her children, Dennis Cook and Brenda Cook; her grandson Brandon Cook; and her siblings, Maude Schoff, Estella Jakie, Daniel Smith and William Batchler.

She is survived by: her children, Lisa Ottinger (Wayne), William Cook and Kenny Cook Jr.; her grandchildren, Justin Ottinger, Austin Cook and Abby Cook; her great grandchildren, Ava, Braxton, Nila, Aubrey, Benson and Karoline; and her siblings, Ruby Rampaio, Alvin Smith, David Smith and Wesley Smith. She is also survived by numerous nieces, nephews.
