She is survived by: her daughter Marsha O'Leary (Tim); her grandchildren, Nicholas Moyer (Tina), Jill Powell (Cory), Jamie Taylor (William) and Chad Powell (Sara); and her great grandchildren, Nicholas Moyer, Christopher Bell, Ava Bell, Kailynn Taylor and Carson Powell.
In lieu of flowers, memorial contributions may be made to American Cancer Society, in Phyllis' memory.
