He was predeceased by: his parents, Nicholas Cook and Lucille Cook (Gauthier). He is survived by: his wife Beverly Cook; his children, Nicholas Cook (Sara), Jeanene Hritz (Dennis Thompson) and Anthony Cook; his grandchildren, David, Beau, Wyatt and Marsalene; and his siblings, Joe Cook (Leslie), Judy Mason and Tim Cook (Sherri). He is also survived by other loving family members and close friends.
