She was predeceased by: her parents, Herbert Leavoy and Angeline Leavoy; and her sister Joann Leavoy. She is survived by: her daughters, Nichole Achatz and Michele Forton; her grandchildren, Dalton Achatz (Heather), Donovan Achatz, Kaya Forton and Andre Forton; her great grandchild Colton Achatz; her brother Clark Leavoy (Sandy); and her nieces, Kim, Kelly and Kerry.
