He was predeceased by: his parents, Harold Maddox and Alma Maddox; his daughter-in-law Sally; and his siblings, Bill Maddox, Peggy Palmer and Johnny Maddox.
He is survived by: his wife Laura "Janie" Maddox; his sons, Dale T. Maddox III (Carrie) of Michigan and Shelby H. Maddox of Georgia; his grandchildren, Brandon Maddox of Georgia, Elena, Debra and Patricia Maddox of Michigan; and his sister Louise Bohnenkamp of Oregon.
