He was predeceased by: his parents, Lambert Cook and Marion Cook (VanHouten); and his sons, David and DJ. He is survived by: his girlfriend Dorothy Koppenol; his children, Debra Bates (Bob), Marianne Talalay (James) and Mark Cook (Lauryn Werschem); his grandchildren, Kelly Joldersma (Kevin), Joel Bush (Delia), Justin Behler, Joshua Talalay, Randy Cook and Crystal Pruett (Nick); his great grandchildren, Oscar, Henry, Ella Joldersma, Calvin, Hannah Bush, Julian, Alec and Gwenna Pruett; and his sister Marilyn Vellenga. He is also survived by many nieces, nephews and cousins.
