He was predeceased by: his father Thomas R. Kubasiak; and his grandparents, Ray, Rosemary Kubasiak, Clayton and Dorothy Lahmeyer.

He is survived by: his mother Denise C. Kubasiak (Lahmeyer) of Bronson; his wife Teresa A. Rogers; his daughters, Kara Kubasiak (Trevor Brown) of Gallup, NM and Katie Beaudry (Zack) of Portage; his siblings, Brett Kubasiak of Bronson, Timothy Shrontz of Auburn, IN and Nicole Spooner (Brian) of Union City; his grandchildren, Brady and Maya Beaudry; and his parents-in-law, Jerry and Irene Rogers. He is also survived by many brothers and sisters-in-law, nieces and nephews.
