He is survived by: his wife Cindy Cook; his daughter Hayley; his son-in-law Nathan Smith of Quincy; his grandchildren, Olivia, Henry and Isaac; his siblings, LaVerne Fay (John) of Cincinnati, OH, Evenell Monica (Bob) of Port Townsend, WA, Louella Hodge (Walt) of Watervliet and Milene Martens (Cindy Shaffer) of Whitehall; his stepson Todd Cook; his mother-in-law Ellie Benjamin; and his sister-in-law Lori Batten of Coldwater.
