He was predeceased by: his parents, Carl and Barbara. He is survived by: his wife Carolyn; his children, Laura, Brian (Michelle) and Michael (Karyn); his grandchildren, Johnny, Matthew, Ella, Andrew (Drew) and Gerald; and his sibling Sandy.
In lieu of flowers, Jerry requested that donations were directed to his daughter's clinical research program.
