He was predeceased by: his father Melvin LaVake. He is survived by: his mother Dorothy LaVake (Witherspoon); his wife Jennifer; his children, Cassie LaVake, Shana Smith, Holly, Emma Ingalls and Joey LaVake; and his siblings, Tom Proctor, Ken Proctor and Dawn. He is also survived by 4 grandchildren.
