She was predeceased by: her father Jerrold Shafor. She is survived by: her mother Julieann Repp; her children, Skyler Larkin (Cambria) and Jenna Burzan (Jared); her grandchildren, Liam, Xavier, Eloise Decker, Addyson Larkin and Julian Burzan; her siblings, Jen Packowski (Mark) and Jerry Shafor II (Amy); her nieces and nephews, Brittany Walters (Troy), Matt Packowski (Lauren), Bethany Packowski (Dan Bryan), Miranda Traver (Marcus), Natalie Mejia (Adrian) and JD Shafor III; her close friend Denise Webb; and her pets, Brody and Charlie.
In lieu of flowers, Jessica requested that memorial contributions be made to either the Cystic Fibrosis Foundation in honor of Addyson (http://www.cff.org) or to the family for the creation of a college trust fund for her grandchildren.
